• Was born in or spent a portion of his/her life in Madison County and has created a positive impact within the county, state, or nation; Mandatory
• Regionally or nationally recognized in his/her field of work;
• Recipient of a major national award.

Information about nominee:	All nominations must include a biographical/historical sketch about the person and why a plaque should be given. If possible, please include a digital copy of a  photograph of the person you are nominating; or, this may be mailed separately to Madison County Historical Society, Heritage Walk of Fame Committee, P. O. Box 397, Richmond, KY 40476.  All items submitted become the property of the Madison County Historical Society. Deadline for sending nominations is April 1st.
